Physical Exams
Physical and wellness exams included FREE for members annually. Non-member physical & labs option available. The annual wellness exam can be a good time to ask your doctor questions about your health or discuss any changes or problems that you have noticed.
Not ready to become a member? Are you looking for a once-a-year physical and wellness exam? Contact us today! Dr. Maxwell provides an option for non-members as well. During this one-hour visit, Dr. Maxwell will review your medical history and family history and will provide you preventive and wellness recommendations. Your labs are included for FREE! Dr. Maxwell will also review the results with you over the phone to help you understand what the results mean.
Chronic Disease Prevention
Reviewing appropriate screening tests, engaging in lifestyle counseling, discussions to prevent chronic conditions.
In order to MAXimize your LIGHT, your Living In God’s Holy Temple, a preventive physical exam is included in your membership each year. We will review appropriate screening tests and immunizations, engage in discussions about lifestyle and prevention of chronic conditions. Immunizations are not provided in our office, but we will provide options for you to complete them.
All members also receive complimentary access to the online course 8 Weeks to a Healthier You created by Dr. Maxwell to help support weight loss and the development of skills for long-term maintenance of keeping the weight off.
Cervical cancer screening
Cervical cancer screening will be recommended to women according to screening guidelines. The lab and pathology fees for pap smears have been discounted as a part of your membership.
Pediatric Services
Pediatric well visits and Sports physicals are included in your child’s membership. We do not provide immunizations in our office, but we will assist in arranging them for your child.

To prevent chronic diseases or catch them early, visit your doctor regularly for preventive services.

- Tobacco use and exposure to secondhand smoke.
- Poor nutrition, including diets low in fruits and vegetables and high in sodium and saturated fats.
- Lack of physical activity.
